Dietary Accommodation Form

Federal law and USDA regulation require nutrition programs to make reasonable modifications to accommodate children with disabilities. Under the law, a disability is an impairment which substantially limits a major life activity or bodily function, which can include allergies and digestive conditions, but does not include personal diet preferences.

The form below is available to make substitutions for the above health concerns.  The form needs to be signed and filled out by a physician.  Once it is received by the Food Service Director, we will contact you on how we can meet your needs.  Milk is not a requirement for School Lunches so this form is not needed in the case of your student not wanting to take a milk.
