Back to School Forms - Island Park, Lakeridge, Northwood and West Mercer
In our continued effort to streamline the myriad of forms that start the school year, Mercer Island School District has implemented a new process: RSVP (Review Student Verification Process)
Beginning August 15, you will be able to log into your Skyward account and complete all the district forms, permission slips and notifications required for the start of school. Parents and guardians will also verify their student demographic information and update phone numbers, email addresses, and emergency contacts. All parents will be required to complete and/or acknowledge all forms for each of their children. The forms included in the process are:
- Demographic information update
- Health Form
- Parent/Student Handbook receipt
- Field Trip Behavior Form
- Permission to Travel to City Parks Form
- Participation in Onlne Curriculum Projects Information
- FERPA Notification
- Student Rights and Responsibilities Notification
- Student Housing Questionnaire
- Verification of Military Family Information
- Attendance Letter
- Student Insurance Letter
Forms should be completed prior to the first day of school for elementary students.
Islander Middle School Forms
In our continued effort to streamline the myriad of forms that start the school year, Mercer Island School District has implemented a new process: RSVP (Review Student Verification Process)
