Back to School Forms

Elementary - Island Park, Lakeridge and West Mercer
In order to simplify the process of providing information and then returning forms with signatures at the start of school, Island Park, West Mercer and Lakeridge are implementing a new process.  Electronic versions of all information is provided below.  On the first day of school, your child will come home with an Acknowledgement Form (also available online soon).  You will indicate acknowledgment and agreement with each item and return just ONE Acknowledgement Form per student.  We hope that reducing the amount of paper and having the information posted all year will make it simpler and more accessible for parents.
 
Elementary Information and Forms - The following forms apply to all elementary students.  Please read and indicate acknowledgement and agreement through the Acknowledgement Form which will come home on the first day of school through back pack mail.
